Transition of care in rCTDs

 

Course overview

The ERN ReCONNET Transition Training in Rare and Complex Connective Tissue Diseases (rCTDs) is a structured educational programme developed by ERN ReCONNET to support healthcare professionals involved in the transition of adolescents and young adults from paediatric to adult care.

The course addresses key clinical, organisational, and psychosocial aspects of transitional care, promoting continuity of care, patient empowerment, multidisciplinary collaboration, and harmonisation of practices across Europe.

 

Course objectives

At the end of the course, participants will be able to:

  • understand developmental, psychological, and social aspects of adolescence and young adulthood
  • recognise disease-specific challenges of rCTDs during transition
  • apply principles of structured transitional care
  • communicate effectively with young people and families
  • address psychosocial, ethical, and legal aspects of transition care
  • support implementation and improvement of transition programmes in clinical practice.

Target audience

Paediatric and adult rheumatologists and other healthcare professionals involved in the management of rare connective tissue diseases, including ERN ReCONNET healthcare provider centres.

 

Programme structure

The course follows a blended learning format consisting of two complementary phases. Phase 1 is a three-day in-person training course held in Ljubljana (Slovenia) on 22–24 October 2026, combining lectures, interactive workshops, case-based discussions, and panel sessions on adolescent medicine, rCTDs in transition, transitional care models, communication skills, and programme implementation. Phase 2 consists of a six-week virtual learning programme with weekly online sessions focused on case discussions, thematic learning, peer exchange, and mentor feedback within the ERN ReCONNET network.

Faculty

The course is delivered by an international, multidisciplinary faculty comprising ERN ReCONNET transition experts, paediatric and adult specialists, and patient representatives.

 

Accreditation

The course is nationally accredited by the Slovenian Medical Chamber. Application for European Accreditation Council for Continuing Medical Education (EACCME) accreditation is ongoing.

 

Registration

Registrations will be opened soon. The deadline to apply for the course is on 2026. Participation is open to healthcare professionals from ERN ReCONNET centres and beyond.

 

Organisation and independence

The course is organised and funded by ERN ReCONNET and is fully independent from pharmaceutical or medical device industry involvement.
