How to access the center:
Patients from Madrid region. Directly sent by their family doctor (“free patient election” from areas not belonging to the hospital)
Patients from other regions. Sent by their family doctor or another specialist (“channeling proposal”)
Cómo acceder al centro:
Si usted vive en la Comunidad de Madrid, puede solicitar que su Médico de Cabecera le envíe al Hospital 12 de Octubre, Servicio de Reumatología, al CSUR de Enfermedades Autoinmunes, a través del sistema de libre elección de especialista
Si usted vive en otra Comunidad Autónoma, puede solicitar a sus médicos especialistas que le envíen al CSUR de Enfermedades Autoinmunes mediante una propuesta de canalización, a la que deben adjuntar un informe sobre su enfermedad, y el motivo de la solicitud de canalización (segunda opinión, evaluación diagnóstica o de posibilidades terapéuticas, etc). Esta propuesta se gestiona a través de la aplicación SIFCO
