ERN ReCONNET Presents Transition of Care Framework at EULAR 2026

ERN ReCONNET is proud to announce that Dr. Mojca Zajc Avramovič (Ljubljana, Slovenia) delivered an oral presentation during the EULAR 2026 Congress in London on Friday, 5 June 2026, showcasing the work carried out by the ERN ReCONNET Transition of Care Task Force.

The presentation, entitled “Consensus Development of the ERN ReCONNET Training Framework for Pediatric-to-Adult Healthcare Transition for Rare Connective Tissue Diseases”, was delivered during the scientific session “Basic and Clinical Abstract Sessions: From A(PS) to C(TD)”.

Presented on behalf of the ERN ReCONNET Transition of Care Task Force, led by Prof. Tadej Avčin and Prof. Gabriele Simonini, the work highlights the collaborative effort undertaken across Europe to develop the first accredited educational course dedicated to the transition from paediatric to adult care in rare connective tissue diseases (rCTDs).

The presentation represents an important milestone in ERN ReCONNET’s commitment to improving healthcare transition pathways and supporting healthcare professionals involved in the care of people living with rare connective tissue diseases.
