In News, News from ERN ReCONNET

The in-person phase of the ERN ReCONNET Accredited Course on Transition of Care in Rare and Complex Connective Tissue Diseases (rCTDs) has successfully concluded in Ljubljana, Slovenia.

Held from 22 to 24 October 2026, the course brought together paediatric and adult rheumatologists, healthcare professionals, and patient representatives to address the challenges associated with the transition of young patients from paediatric to adult care. Organised by Tadej Avcin (Ljubljana) together with Prof. Gabriele Simonini (Florence), and supported by the ERN ReCONNET Task Force on Transition of Care, the training provided participants with theoretical insights and practical tools to implement structured transition pathways in clinical practice.

The interactive sessions and practical workshops focused on key strategic areas:

– Transition models and planning: Standardised frameworks to ensure continuity of care and assess patient readiness.

– Communication strategies: Effective approaches for engaging with adolescents, young adults, and their families on complex clinical and psychosocial topics.

– Clinical application: Integration of digital tools and multidisciplinary practices into daily healthcare routines.

ERN ReCONNET extends its gratitude to the international faculty, expert speakers, and participants for their active contribution and commitment throughout the meeting.

Next Steps: The Virtual Learning Programme

The educational initiative now progresses into its second phase with the launch of the six-week virtual learning programme. This online module will feature interactive case discussions, thematic sessions, peer exchange, and dedicated mentor feedback across the Network. Together with the in-person component, the complete course is EACCME® accredited for up to 21.0 European CME credits (ECMEC®s).

Through initiatives like this, ERN ReCONNET continues its mission to standardise care practices, empower healthcare professionals, and enhance care pathways for young people living with rCTDs across Europe. 💙
