ERN ReCONNET Transition of Care accredited course to be featured in EURORDIS webinar series
The ERN ReCONNET’s accredited Transition of Care Course will be presented during the EURORDIS Patient Partnership Webinar Series “Transition in Healthcare – Turning Recommendations into Practice”, taking place on 1 July 2026.
The webinar will showcase innovative initiatives developed across the ERNs to improve the transition from paediatric to adult healthcare for people living with rare diseases. During the session, Silvia Aguilera (ePAG Co-Chair in ERN ReCONNET) and Mojca Zajc Avramovič (Clinical lead in University Medical Centre Ljubljana, ERN ReCONNET Member) will present the development of the ERN ReCONNET Transition of Care Course, highlighting its role in supporting healthcare professionals and promoting high-quality transition practices.
The course is one of the key outputs of the ERN ReCONNET Transition of Care Task Force, established in 2023 and bringing together paediatric and adult clinicians, patient representatives, ERN ReCONNET Young members, and the Coordination Team. The Task Force is working to identify unmet needs in transition care for young people with rare connective tissue diseases (rCTDs), develop recommendations, and create educational resources that support a more coordinated, patient-centred transition across Europe.
